About the Role
Vancouver Coastal Health is seeking a skilled Wellness Support Worker to join our team in Bella Coola, BC. As a member of our interdisciplinary community health team, you will provide a variety of day-to-day physical, emotional, and social supports to designated population groups in the community.
Key Responsibilities
- Work collaboratively with healthcare professionals to provide comprehensive care to clients.
- Develop and implement individualized support plans to promote client independence and well-being.
- Provide education and resources to clients and their families on health-related topics.
- Establish and maintain relationships with community agencies and organizations to ensure seamless care coordination.
- Participate in multidisciplinary team meetings to discuss client progress and develop care plans.
Requirements
- Completion of a certificate in a Community Social Service or related program and two years of recent, related experience working with designated population groups.
- Valid class V BC Driver's license for worksites that require driving to various locations.
- Ability to communicate effectively with clients, families, and healthcare professionals.
- Ability to work independently and as part of a team.
- Ability to maintain confidentiality and handle sensitive information.
