Hours of Work: 21 hours per week
Reports to: Collaborative Practice and Care Services Manager
Position Summary:
This role involves engaging clients/patients who are experiencing mental health challenges and/or addictions, facilitating experiential learning of life skills, and supporting their recovery journey. The peer support worker will also assist the Care Services teams by participating in outreach and group activities.

Responsibilities:
1. Provides support and empowers clients or patients facing mental health conditions and/or addictions Assists and motivates clients or patients to live their best lives through individual support and by developing, organizing, and facilitating group activities While respecting client/patient autonomy, collaborates with Care Managers and GAIN team members to implement care plans and recommendations. 2. Provides departmental and organizational support Engages in all aspects of outreach initiatives. Organizes, facilitates, and/or presents educational sessions on mental health and/or addictions to raise awareness and combat stigma Researches and prepares materials for support and/or educational groups, including presentation resources and refreshments, as necessary Aids the team in supporting volunteers and student placements Experience & Skills: One to two (1-2) years' experience in health/social services, providing support to individuals with addictions from a harm reduction perspective Personal experience with the mental health and addictions system and a continuous recovery journey Experience working with older adults and familiarity with geriatric syndromes is an asset Peer Support Certification and CPR Certification are advantageous Strong interpersonal, written, and verbal communication skills Ability to communicate in a second language is a plus Others: A valid driver's license and access to a vehicle for home visits are required Availability to work occasional evenings and weekends is necessary Vulnerable Sector screening is mandatory Proof of full COVID-19 Vaccination or Medical Exemption Certificate is required Senior Persons Living Connected promotes a diverse work environment.
