Nova Scotia Health Authority is seeking a dedicated and compassionate Health Care Social Worker to join our team in the Western Zone.

As a vital member of the interdisciplinary healthcare team, you will provide holistic care across the continuum, from primary to quaternary levels. Your expertise will focus on the intricate relationship between individuals and their environments, recognizing the profound impact these connections have on psychosocial, emotional, and spiritual well-being.

You will leverage your knowledge of social determinants of health to guide a dynamic and interactive process of engagement, assessment, planning, intervention, and evaluation. By empowering patients and families, you will build upon existing coping strategies, problem-solving skills, past successes, and personal resources.

About You
  • You hold a Bachelor's degree in Social Work from a recognized university.
  • You are currently registered with the Nova Scotia College of Social Workers.
  • You possess 2 years of post-BSW related clinical experience.
  • Previous experience working in a community-based setting is an asset.
  • You demonstrate a strong commitment to a client-/family-centered approach.
  • You are familiar with various evidence-based treatment approaches, including Motivational Interviewing and behavior change counseling.
  • You have experience facilitating group sessions.
  • You can effectively complete biopsychosocial assessments.
  • You possess excellent communication skills.
  • You are adept at assisting patients and families in navigating local community resources and agencies.
  • You hold a valid Nova Scotia Driver's license and access to a reliable vehicle.
  • You demonstrate strong organizational and time management abilities.
