About the Role

We are seeking a dedicated and compassionate Social Work Professional to join our team. In this role, you will provide comprehensive social work services to patients, families, and caregivers facing a variety of health challenges.

Responsibilities

  • Conduct comprehensive assessments of patient needs, including psychosocial, emotional, and environmental factors.
  • Develop and implement individualized care plans that address the unique needs of each patient.
  • Provide counseling and support to patients and families coping with illness, injury, or loss.
  • Advocate for patients' rights and access to resources.
  • Collaborate with a multidisciplinary team of healthcare professionals to ensure coordinated and holistic care.
  • Educate patients and families about available community resources and support services.

Qualifications

  • Master's degree in Social Work (MSW) from an accredited program.
  • Current registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Experience working in a healthcare setting, preferably with a focus on acute care or complex medical conditions.
  • Strong clinical assessment and intervention skills.
  • Excellent communication, interpersonal, and problem-solving abilities.
