About the Role:
We are seeking a highly skilled and experienced Forensic Community and Liaison Social Worker to join our team at BC Mental Health and Substance Use Services. As a key member of our team, you will be responsible for providing comprehensive social work services to clients with complex mental health and substance use challenges.
Key Responsibilities:
- Conduct initial intake and ongoing biopsychosocial assessments to identify client needs and develop integrated treatment plans.
- Provide community case coordination and advocacy services to clients, including referrals to other health and social services.
- Maintain accurate and concise documentation of client information and provide written reports as required.
- Collaborate with other healthcare professionals to ensure comprehensive care and support for clients.
- Facilitate access to psychiatric and social services through referrals and site visits.
Requirements:
- Bachelor's degree in Social Work from an accredited school.
- Two years of recent experience working in a mental health setting.
- Full registration with the British Columbia College of Social Workers (BCCSW).
- Valid BC Driver's License.
- Current Cardiopulmonary Resuscitation (CPR) Certificate.
