Job Title: Forensic Community and Liaison Social Worker

BC Mental Health and Substance Use Services is seeking a highly skilled and compassionate Forensic Community and Liaison Social Worker to join our team in Kamloops, BC.

Job Summary:

We are looking for a dedicated and experienced social worker to provide community-based services to individuals with complex mental health and substance use challenges. As a Forensic Community and Liaison Social Worker, you will work closely with clients, families, and community resources to develop and implement individualized treatment plans, provide advocacy and support, and facilitate access to psychiatric and social services.

Key Responsibilities:
  • Conduct initial intake and ongoing biopsychosocial assessments to identify client needs and develop integrated treatment plans.
  • Provide community case coordination and advocacy services, collaborating with other health and social service providers to ensure comprehensive care.
  • Maintain accurate and concise documentation of client information, providing written reports and summaries as required.
  • Facilitate access to psychiatric and social services through referrals, site visits, and home visits, as needed.
  • Consult with clinical treatment teams to determine client needs and develop strategies for community reintegration and prosocial development.
Requirements:
  • Bachelor's degree in Social Work from an accredited School of Social Work.
  • Two years recent, related experience working in a mental health setting.
  • Full registration with the British Columbia College of Social Workers (BCCSW) is required.
  • Valid BC Driver's License and access to a reliable vehicle.
  • Current Cardiopulmonary Resuscitation (CPR) Certificate.
