Position Summary: The Clinical Coach at VON Canada plays a pivotal role in facilitating practice enhancements aimed at improving access to high-quality clinical services in accordance with the Palliative Care Health Services Delivery Framework and the Ontario Palliative Care Competency Framework.
Key Duties:
- Ensure community initiatives align with the Ontario Palliative Care Network's guidelines.
- Promote equity, inclusion, and diversity within Ontario's healthcare landscape.
- Provide mentorship and guidance on palliative care practices to staff and clinicians within community organizations.
- Collaborate with leadership on local service integration and change management strategies.
- Consult on complex clinical care for patients with diverse needs.
- Lead engagement initiatives with partner organizations.
- Support Indigenous communities in their implementation efforts.
- Work with regional partners to execute the Delivery Framework implementation plan.
- Contribute to educational and system integration initiatives.
- Keep regional teams informed about community challenges and advancements.
- Facilitate feedback and assist in data collection for performance measurement and reporting.
- Communicate essential messages to various stakeholder groups.
- Assist in planning for Ontario Health Teams and developing palliative care models.
- Provide educational resources to community partners regarding palliative care.
- Assess and guide quality improvement efforts within community organizations.
- Mentor healthcare providers and spearhead practice change initiatives.
General Responsibilities:
- Advocate for VON's mission and values as a community care provider.
- Ensure a safe working environment in compliance with health and safety standards.
- Adhere to all organizational policies and procedures.
- Foster internal and external relationships to enhance communication.
- Engage with diverse communities in the realm of palliative care.
Qualifications and Experience:
- Degree in a recognized healthcare discipline.
- At least five years of experience in palliative care.
- Experience in research, program evaluation, and quality improvement projects.
- Familiarity with various healthcare roles and environments.
- Expertise in palliative care consultation and regional healthcare priorities.
Skills Required:
- Strong clinical coaching, consultation, and analytical skills.
- Ability to perform under pressure and collaborate effectively in a team.
- Proactive problem-solving and negotiation capabilities.
- Excellent communication and relationship-building skills.
- Knowledge of educational technologies and a person-centered approach to care.
Additional Requirements:
- Access to reliable transportation for travel between organizations.
- Willingness to travel as necessary.
- Successful completion of a Criminal Records Check is mandatory.
