Employment Type: Permanent Part Time

The Care Coordinator plays a crucial role in establishing and maintaining effective relationships between the clinic and its clientele, which includes patients, staff, funding agencies, and referral sources. This is accomplished by delivering exceptional administrative support and managing clinic operations with a warm, enthusiastic, and professional demeanor towards all patients, team members, and external stakeholders.

The Care Coordinator oversees all facets of patient onboarding, data management, telephone communications, outbound outreach, as well as processing invoices and payments.

Key Responsibilities

  • Manage incoming calls, warmly welcome patients and visitors upon their arrival, and deliver outstanding customer service for any inquiries or issues
  • Ensure a thorough and precise collection of information during the patient's initial visit and throughout their treatment journey
  • Assist patients in completing necessary documentation at the clinic to minimize errors and ensure accuracy
  • Organize and schedule patient appointments while maintaining accurate medical records
  • Collect payments at the time of service delivery
  • Monitor coverage for all funding sources to guarantee approval
  • Execute data entry and billing tasks efficiently
  • Coordinate the handling of courier packages (incoming and outgoing) and manage email, mail, and fax communications
  • Provide administrative assistance to the Care Coordinator Manager and Clinic Director

Required Qualifications & Core Competencies

  • High School Diploma, G.E.D. or equivalent
  • Strong customer service abilities and telephone communication skills
  • High level of organizational capabilities with the ability to prioritize and manage multiple tasks
  • Excellent verbal and written communication skills, along with strong interpersonal abilities
  • Proficient computer skills and data entry experience
  • Adaptable and flexible in response to changing circumstances
  • Previous experience in a Physiotherapy clinic is considered an asset
