Youth Care Coordinator

Job Summary
The Centre for Addiction and Mental Health is seeking a skilled Registered Nurse to join our team in the Child, Youth and Family Services department. As a Nurse Specialist, you will provide high-quality care to youth with complex mental health and addiction issues.
Key Responsibilities
- Provide client-centered psychiatric, addictions and general nursing care to youth with concurrent disorders
- Develop and implement individualized care plans in collaboration with the interprofessional team
- Conduct mental status assessments, monitor patient response to interventions, and provide quality clinical documentation
- Support clients in establishing and working towards their goals, and participate effectively in interprofessional collaboration and teamwork
- Liaise with other services at CAMH and community agencies to coordinate care for clients
Requirements
- Registered and in good standing with the College of Nurses of Ontario
- Current B.C.L.S. (CPR) certificate
- Bachelor's degree in nursing, BScN
- Preference given to RNs with certification in Psychiatric/Mental Health Nursing through the Canadian Nurses Association (CAN)
- Specialized clinical experience working with complex and challenging youth and their families
