Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Community Health Specialist. As a key member of our healthcare team, you will provide medical care to children, youth, adults, and seniors in their homes and other community settings across Nova Scotia.

Key Responsibilities

  • Provide comprehensive medical care, including diabetes management, dialysis, IV medications, IV/CVAD care, palliative care, phlebotomy, and wound care, including negative pressure wound therapy and complex wounds.
  • Develop and implement individualized care plans that address the unique needs and goals of each client.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless care coordination.
  • Conduct thorough assessments, identify risks, and develop strategies to mitigate them.
  • Provide education and support to clients and their families on disease management, medication adherence, and healthy lifestyle choices.
  • Participate in quality improvement initiatives and contribute to the development of best practices in community health care.

Requirements

  • Current registration with the College of Registered Nurses of Nova Scotia.
  • Experience in community health care, preferably in a home care or hospice setting.
  • Valid driver's license and access to a reliable vehicle.
  • Ability to wear personal protective equipment (PPE) and work in a fast-paced, dynamic environment.
