Nurse Practitioner

We are seeking a dedicated and compassionate Registered Nurse to join our team in Primary Care at Northern Health Authority.
Job SummaryThe successful candidate will provide high-quality patient care, work collaboratively with the interprofessional team, and contribute to the development of care plans that meet the unique needs of our patients.
Responsibilities- Provide direct clinical care and services to individuals, groups, and communities
- Develop and implement care plans that address the physical, emotional, and social needs of patients
- Collaborate with the interprofessional team to ensure seamless care transitions and coordination
- Participate in education and training programs to enhance skills and knowledge
- Contribute to quality improvement initiatives and patient safety efforts
- Registration with BC College of Nurses and Midwives as a practicing RN or RPN registrant
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings
- Current valid BC Driver's License
