Job Title: Care Coordinator

Job Summary: We are seeking a highly skilled Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in assessing and determining patient care needs, providing access and referrals to community services, and engaging with patients, caregivers, and other healthcare practitioners.

Key Responsibilities:

  • Assess and determine patient care needs and eligibility
  • Provide access and referrals to community services
  • Engage with patients, caregivers, and other healthcare practitioners
  • Develop and implement individual care and service plans
  • Coordinate and monitor care plan delivery

Requirements:

  • Membership in good standing with the applicable regulatory body
  • 2+ years of recent experience in community health or a related field
  • Knowledge of the healthcare delivery system and community resources
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
  • Effective time management, prioritization, and organizational skills
