Job Summary

We are seeking a highly motivated and experienced Rehabilitation Service Worker to join our team. The successful candidate will be responsible for developing and implementing rehabilitation programs, conducting assessments, and providing support to clients.

Key Responsibilities
  • Develop and implement rehabilitation programs for clients with various needs and goals.
  • Conduct assessments to identify client needs and develop individualized plans.
  • Provide support and guidance to clients in achieving their rehabilitation goals.
  • Liaise with other social services agencies and healthcare providers to ensure comprehensive care.
  • Maintain accurate records and statistics to evaluate program effectiveness.
  • Assist in evaluating the effectiveness of treatment programs and making recommendations for improvement.
Requirements
  • College/CEGEP education in a related field.
  • 3-5 years of experience in a rehabilitation setting.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong organizational and time management skills.
