Job Title: Rehabilitation Service Worker

We are seeking a highly skilled and dedicated Rehabilitation Service Worker to join our team at Chezranti Community Centre Ltd.

Job Summary:

The Rehabilitation Service Worker will be responsible for providing rehabilitation services to clients, including developing and implementing individualized rehabilitation plans, conducting assessments and evaluations, and providing support and guidance to clients and their families.

Key Responsibilities:
  • Develop and implement individualized rehabilitation plans for clients
  • Conduct assessments and evaluations to identify client needs and goals
  • Provide support and guidance to clients and their families
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Maintain accurate and up-to-date records of client progress and services provided
  • Participate in team meetings and contribute to the development of rehabilitation programs
Requirements:
  • 3 years to less than 5 years of experience in a rehabilitation setting
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
  • Ability to maintain confidentiality and handle sensitive information
Work Environment:

The Rehabilitation Service Worker will work in a fast-paced environment with a diverse client population. The ideal candidate will be able to work independently and as part of a team, and will have excellent communication and interpersonal skills.
