Job Summary

We are seeking a highly skilled and compassionate PACT Service Coordinator to join our team at the Winnipeg Regional Health Authority. As a key member of our multidisciplinary team, you will play a vital role in assessing, planning, and coordinating treatment, rehabilitation, and support services for individuals with serious and persistent psychiatric disabilities.

Key Responsibilities
  • Assess and develop individualized treatment plans in collaboration with the clinical team
  • Provide psychosocial rehabilitation and recovery-oriented services to program participants
  • Coordinate and facilitate multidisciplinary team meetings and case conferences
  • Develop and maintain relationships with community agencies and service providers
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures
Requirements
  • Minimum four years of related community experience working with individuals with serious and persistent psychiatric disabilities
  • BSW, OT, PT, BSc PN, RPN, BN, RN, SLP, RD, or related health/human service degree required
  • Working knowledge of the principles of psychosocial rehabilitation and recovery
  • Effective networking and agency relation skills
  • Excellent organizational, communication, and interpersonal skills
