We are seeking a highly skilled and compassionate PACT Service Coordinator to join our team at the Winnipeg Regional Health Authority. As a key member of our multidisciplinary team, you will play a vital role in assessing, planning, and coordinating treatment, rehabilitation, and support services for individuals with serious and persistent psychiatric disabilities.
Key Responsibilities- Assess and develop individualized treatment plans in collaboration with the clinical team
- Provide psychosocial rehabilitation and recovery-oriented services to program participants
- Coordinate and facilitate multidisciplinary team meetings and case conferences
- Develop and maintain relationships with community agencies and service providers
- Participate in quality improvement initiatives and contribute to the development of policies and procedures
- Minimum four years of related community experience working with individuals with serious and persistent psychiatric disabilities
- BSW, OT, PT, BSc PN, RPN, BN, RN, SLP, RD, or related health/human service degree required
- Working knowledge of the principles of psychosocial rehabilitation and recovery
- Effective networking and agency relation skills
- Excellent organizational, communication, and interpersonal skills
