  • Individual, group, and family treatment
  • Risk screening and counseling
  • Case management and coordination
  • Transition and discharge planning
  • Prevention, promotion, and public awareness activities

Key Responsibilities:

As a Social Worker II at Alberta Health Services, you will:

  • Work collaboratively with healthcare teams to provide comprehensive patient care
  • Conduct thorough psychosocial assessments to identify patient needs and develop effective intervention plans
  • Provide individual, group, and family treatment to address patient concerns and promote healthy behaviors
  • Develop and implement case management plans to ensure seamless transitions and optimal patient outcomes
  • Collaborate with community resources and healthcare providers to ensure comprehensive patient care

Requirements:

To be successful in this role, you will require:

  • A bachelor's degree in Social Work
  • Active or eligible registration with the Alberta College of Social Workers (ACSW)
  • Knowledge of relevant legislation, including the Personal Directives Act, the Adult Guardianship and Trusteeship Act, and the Child and Youth Family Enhancement Act
  • Effective time management and excellent written and verbal communication skills

Preferred Qualifications:

Experience in grief and loss, as well as knowledge of community resources and financial supports, would be beneficial in this role.
