Job Summary
The Community Support Worker III plays a vital role in the Menatl Health and Addictions Services team at Prairie Mountain Health. This position requires a client-centered approach to help individuals maintain wellness in the community, supporting the goals of the client within the principles of quality care.
Key Responsibilities
- Provides a holistic approach to strengthen client skills, promoting physical, psycho-social, and spiritual well-being.
- Encourages client independence, self-confidence, and dignity by assisting with daily living activities.
- Assists clients in managing their environment, promoting active participation in their care.
- Develops a broader network of support for clients and families, connecting them with community resources.
- Conducts self in a professional, tactful, and courteous manner when interacting with staff, clients, families, and volunteers.
- Schedules activities and programming to meet client needs, in consultation with the supervisor.
- Assists in the orientation of new staff and students as requested.
Individualized Supports to Clients and Families
- Works with the supervisor and other team members to develop an appropriate plan of care, taking into account client strengths and needs.
- Conducts appointments and home visits in a professional and ethical manner, building a therapeutic working relationship with clients.
- Assists clients in linking with existing community resources, promoting healthy living initiatives.
- Performs other duties as assigned by the supervisor.
