Vancouver Coastal Health (VCH) is seeking a compassionate Community Care Assistant to enhance our services in the community. In this role, you will be instrumental in delivering exceptional support to clients and patients.
Key Responsibilities:
- Deliver personalized home support services to clients under the guidance of the Supervisor/Coordinator/Manager.
- Assist clients with their care requirements and monitor any notable changes in their condition.
- Work collaboratively with the healthcare team to promote client autonomy.
- Engage in care planning and provide constructive feedback to the team.
Required Qualifications:
Education & Experience
- Completion of a recognized Health Care Assistant Program.
- Registration with the BC Community Health Worker and Care Aide Registry.
- Experience with elderly and palliative care populations is preferred.
- Possession of a valid BC Driver's license and access to a vehicle.
