We are seeking a highly skilled Nurse Practitioner to collaborate with our Director of Care in supporting and aligning resident care at peopleCare Communities.
Key Responsibilities- Collaborate with the Physician, Director of Care, and leadership team to deliver high-quality care.
- Assess, diagnose, and evaluate resident conditions, prescribing treatments and diagnostics as needed.
- Develop, implement, and evaluate resident care plans.
- Act as a liaison with physicians, supervisors, and other departments to ensure seamless care delivery.
- Provide clinical leadership and education to nursing and personal support care team members.
- Monitor staff performance and participate in identifying trends in resident care and professional nursing practice.
- Participate in the organization's Quality Improvement Plan (QIP) and contribute to research and best practices.
- Nurse Practitioner Certification from a recognized program.
- Current registration with the College of Nurses of Ontario as Registered Nurse in the Extended Class or RN (EC), in good standing.
- Advanced knowledge and decision-making skills in assessment, diagnosis, and health care management.
- Experience in Long-Term Care considered an asset.
- Excellent leadership, communication, and interpersonal skills.
