Nurse Practitioner

The Nurse Practitioner will collaborate with the Director of Care to support and align resident care at peopleCare Communities. As a member of the Care Management team, this position provides comprehensive nursing care and directs the work of nursing and personal care staff in the assigned unit.
Key Responsibilities- Collaborate with the Physician, Director of Care, and leadership team to deliver high-quality care.
- Assess, diagnose, and evaluate acute and chronic illnesses, and develop treatment plans.
- Prepare, implement, review, and evaluate resident care plans.
- Act as a liaison with physicians, supervisors, other departments, government inspectors, consultants, social workers, clergy, relatives, or representatives to ensure appropriate resident care.
- Provide clinical leadership and education to nursing and personal support care team members.
- Monitor the performance of nursing and personal care staff.
- Participate in identifying, analyzing, and interpreting trends in resident care and professional nursing practice.
- Site Wound Care Champion.
- Participate in scheduled and required assessment processes.
- Monitor resident health and note any changes in conditions.
- Maintain resident records in compliance with established standards and legislative requirements.
- Participate in the orientation and training of new personnel and students.
- Participate in committees/teams/task forces as required.
- Participate in the organization's Quality Improvement Plan (QIP) utilizing research and best practices.
- Organize and direct functions and activities related to the provision of resident care.
- Delegate clinical and administrative responsibilities to the appropriate staff.
- Nurse Practitioner Certification from a recognized program.
- Current registration with the College of Nurses of Ontario as Registered Nurse in the Extended Class or RN (EC), in good standing.
- Maintain competencies as per the governing College standards.
- Current Membership with the Registered Nurses Association of Ontario (RNAO).
- Advanced knowledge and decision-making skills in assessment, diagnosis, and health care management.
- Experience in Long-Term Care considered an asset.
- Experience working with seniors and unregulated care providers.
- Education and experience in gerontology, rehabilitation, and community/complex/long-term care an asset.
- Knowledge of Infection Control and Risk Management Practices.
- Proven leadership abilities with excellent written and oral communication, critical thinking, interpersonal skills, care coordination, time management, and customer focus skills.
- Demonstrated sound clinical knowledge and ability to delegate to unregulated care providers.
- Skills in use of computer and Microsoft Office applications.
