The STAR Family Health Team provides comprehensive primary health care services to two clinics in Stratford, and one clinic to serve Tavistock And Region (STAR). **Description**

We are looking for a **temporary** **part-time Registered Dietitian **to work collaboratively with all members of our multidisciplinary team.You will be located in Tavistock and provide client centered, goal-oriented assessment, teaching and counseling services to patients requiring assistance with nutritional management. The services of this dietitian are particularly important in the area of chronic disease management and prevention/health promotion.

**Qualifications**:
Must have experience in group facilitation, diabetes, lipid and weight management and the ability to practice independently. Two years of experience in nutrition counselling is preferred. Required abilities include time management, flexibility and ability to work as a member of a multidisciplinary team. Knowledge and experience with Chronic Disease Management strategies, patient self -management and the Expanded Chronic Care model are required. Certification as a Diabetes Educator and/or Craving Change Facilitator would be an asset. An interest in diabetes management is essential. Experience in eating disorder management would also be an asset. Must be a member in good standing with the College of Dietitians of Ontario, Dietitians of Canada and hold professional liability insurance.

Contract length: 12 months

Pay: $34.00-$36.36 per hour

Expected hours: 24 per week
