Job Summary

Pro Vita Care Management is seeking a skilled Licensed Practical Nurse (LPN) to join our team in a nursing home setting. As a LPN, you will be responsible for providing high-quality patient care, working collaboratively with our healthcare team to ensure the best possible outcomes for our residents.

Key Responsibilities
  • Provide direct patient care to residents in a long-term care setting, including administering medications, conducting specimen collection, and monitoring vital signs.
  • Assist with surgical procedures by laying out instruments, setting up equipment, and passing instruments to surgeons.
  • Develop and implement individualized care plans in collaboration with the healthcare team to meet the unique needs of each resident.
  • Monitor and maintain accurate records of patient care, including medication administration, vital signs, and treatment plans.
  • Collaborate with the healthcare team to ensure seamless communication and coordination of care.
  • Participate in quality improvement initiatives to enhance the quality of care and services provided to residents.
Requirements
  • Eligibility for licensure as a practical nurse in the province/territory of work.
  • Completion of the Canadian Practical Nurse Registration Examination.
  • CPR Certificate.
  • First Aid Certificate.
  • Geriatrics experience an asset.
