Wellness Support Professional

Vancouver Coastal Health is seeking a skilled Wellness Support Worker to join our team in Bella Bella, BC. As a member of our interdisciplinary community health team, you will provide essential physical, emotional, and social supports to designated population groups in the community.
Key Responsibilities- Work collaboratively with healthcare professionals to provide comprehensive care to clients.
- Develop and implement individualized support plans to promote client independence and well-being.
- Provide education and resources to clients and their families on health-related topics.
- Establish and maintain effective relationships with community agencies and organizations.
- Participate in multidisciplinary team meetings to discuss client progress and develop care plans.
- Completion of a certificate in a Community Social Service or related program.
- Two years of recent experience working with designated population groups or an equivalent combination of education and experience.
- Valid class V BC Driver's license for worksites that require driving.
