Job Title: Nurse Practitioner

At Rockcliffe Community, we are seeking a skilled and compassionate Nurse Practitioner to join our team. As a critical role within our community, this position has a significant impact on the lives of our residents and ignites the warmth of human connection.

Key Responsibilities:
  • Provide primary care in accordance with established policy and within scope of practice.
  • Collaborate with physicians to provide follow-up on diagnosed conditions and provide on-call support.
  • Provide nursing consultation to residents, families, health professionals, and administration.
  • Participate in identifying, analyzing, and interpreting trends in resident care outcomes and professional nursing practice issues.
  • Respond promptly to concerns of residents and family members, mediating satisfactory responses and implementing action plans.
Requirements:
  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class.
  • Demonstrated experience in gerontology.
  • Advanced knowledge and decision-making skills in assessment, diagnosis, and health care management.
  • Knowledgeable about primary health care issues in Long Term Care.
  • Strong interpersonal and leadership skills.
  • Successful completion of a Vulnerable Sector Check.
Preferred Qualifications:
  • Certification from the Canadian Nurses Association.
