Nurse Practitioner Role

We are seeking a skilled Nurse Practitioner to join our team at Hawthorn Woods Community. As a critical role within our community, this position will impact the lives of residents and ignite the warmth of human connection.
Key Responsibilities:- Provide primary care in accordance with established policy and within scope of practice.
- Collaborate with physicians to provide follow-up on diagnosed conditions and provide on-call support.
- Provide nursing consultation to residents, families, health professionals, and administration.
- Participate in identifying, analyzing, and interpreting trends in resident care outcomes and professional nursing practice issues.
- Respond promptly to concerns of residents and family members, mediate satisfactory responses, and implement action plans to avoid reoccurrence.
- Perform other duties as assigned.
- Current registration with the College of Nurses of Ontario as Registered Nurse in the Extended Class or RN (EC).
- Demonstrated experience in gerontology.
- Advanced knowledge and decision-making skills in assessment, diagnosis, and health care management.
- Knowledgeable about primary health care issues in Long Term Care.
- Strong interpersonal and leadership skills.
- All applicants must successfully pass the prescribed Vulnerable Sector Check.
- Certification from the Canadian Nurses Association preferred.
