Job Overview:

The Healthcare Services Coordinator (HSC) is a pivotal nursing role within a community environment, tasked with advanced responsibilities in patient care and the facilitation of clinical services. The HSC operates as a vital member of a collaborative healthcare team, delivering services and initiatives that foster wellness, sustain health, and avert illness. This is achieved through comprehensive nursing evaluations, health promotion, and educational counseling, alongside collaboration with fellow team members. The HSC position encompasses additional responsibilities beyond the standard duties of a nurse.

Key Responsibilities:

The following responsibilities illustrate the extensive scope of the role, though they are not exhaustive.

Core – Healthcare Services Coordinator Responsibilities

  • Oversee the collection of patient specimens.
  • Manage vaccine inventory and medication supplies.
  • Serve as the primary contact for clinical service equipment.
  • Assist in the development of policies, procedures, and care directives.
  • Collaborate with the Primary Care Manager to address patient care issues.
  • Act as the liaison for clinical student placements and preceptors.
  • Lead clinical practice meetings.
  • Be the first point of contact for laboratory-related inquiries.
  • Handle payroll for clinic nurses, staff, and allied health professionals.
  • Engage in data collection and program evaluation as necessary.
  • Coordinate consults and specialty clinics (e.g., internal medicine, gynecology).
  • Participate in organizational committees as nominated or deemed appropriate.

Assessments & Interventions:

  • Collaborate effectively with all healthcare team members and other care providers.
  • Deliver health promotion and preventive care for patients and families across all age groups.
  • Provide counseling on contraception and safe sexual practices.
  • Facilitate prenatal care and education.
  • Administer testing, education, and interventions aimed at preventing STBBIs.
  • Offer tailored health education and promotion alongside nursing interventions for individuals and groups managing chronic conditions.
  • Provide advocacy, counseling, support, and referrals for various health and social issues.
  • Support and advocate for patients with complex healthcare needs.
  • Deliver appropriate nursing interventions for both urgent and non-urgent care across all age demographics, from newborns to the elderly.

Nursing Procedures Include:

  • Conduct routine nursing assessments for both healthy and unwell clients.
  • Perform well-baby and child assessments and administer childhood vaccinations.
  • Provide wound care.
  • Conduct testing and treatment for STBBIs.
  • Manage dressings and treatment for minor injuries.
  • Offer treatment advice for minor urgent care situations.
  • Administer cryotherapy for skin conditions/warts.
  • Perform venipuncture and manage the collection and forwarding of specimens.
  • Conduct ear syringing and administer injections, including long-term psychotropic medications.
  • Perform pap tests once adequately trained.
  • Provide foot care (if certified).
  • Administer over-the-counter medications.
  • Implement treatment plans in accordance with established care directives.
  • Administer immunizations throughout the lifespan.

Qualifications:

  • Bachelor of Nursing or Bachelor of Science in Nursing; must be registered with the College of Registered Nurses of Nova Scotia.
  • A minimum of 3 years of experience in community health or a comparable setting.
  • Experience working with underserved populations is preferred.
  • Current CPR certification, Nonviolent Crisis Intervention training, or a willingness to undergo training upon hiring.

Job Type: Full-time

Compensation: $38.10-$44.69 per hour

Expected Hours: 30 hours per week
