Nova Scotia Health is seeking a dedicated and compassionate Health Care Social Worker to join our team in the Western Zone.

As a vital member of the interdisciplinary healthcare team, you will provide holistic care across the continuum, from primary to quaternary levels. Your expertise will focus on the intricate relationships between individuals and their environments, recognizing their profound impact on psychosocial, emotional, and spiritual well-being.

Your Role:

  • Collaborate with a diverse team of healthcare professionals to deliver comprehensive patient care.
  • Conduct thorough biopsychosocial assessments to understand patients' unique needs and challenges.
  • Develop and implement individualized treatment plans, utilizing evidence-based approaches such as Psychosocial Rehabilitation and Motivational Interviewing.
  • Facilitate group sessions to support patients in developing coping mechanisms and building social connections.
  • Advocate for patients and connect them with relevant community resources and agencies.
  • Maintain accurate documentation and contribute to a collaborative and supportive work environment.

About You:

We are looking for an individual who possesses:

  • A Bachelor's degree in Social Work from a recognized university.
  • Current registration with the Nova Scotia College of Social Workers.
  • Two (2) years of post-BSW clinical experience, preferably in adult community mental health.
  • Strong knowledge of the Diagnostic & Statistical Manual of Mental Disorders.
  • Exceptional communication and interpersonal skills.
  • A client-centered approach and a commitment to patient empowerment.
  • The ability to work effectively both independently and as part of a team.
