Halton Healthcare is seeking a skilled Nurse Specialist to provide high-quality patient care in our active complex continuing care unit. As a key member of our healthcare team, you will be responsible for coordinating care, educating patients and families, and supporting factors that impact customer satisfaction.
Key Responsibilities- Provide specialized patient care, including administering medications and procedures, completing health assessments, and documenting observations and changes in patient condition.
- Collaborate with healthcare team members to update individualized care plans and facilitate positive patient outcomes.
- Assume responsibility for the coordination of care, focusing on patient transition through the continuum of care.
- Support patient and family education, and address factors that impact customer satisfaction.
- Current Registration in good standing with the College of Nurses of Ontario.
- Current Heart & Stroke Approved BLS Provider Card upon hire.
- Proven ability to care for patients with tracheostomies, enteral/parenteral feeds, CADD pumps, PICC lines, IVs, Wound Management Systems, Fecal Management Systems.
- Completion of a physical health assessment course.
- One year nursing experience in Long Term Care setting.
- Exceptional communication skills, verbal, written, and electronic.
- Demonstrated compassion, patience, and empathy.
- Demonstrated leadership ability.
- Ability to multi-task and prioritize depending on changing patient needs.
- Ability to work collaboratively in a team environment and independently as needed.
- Computer skills/CCRS data entry an asset.
- Evidence of dedication to continuing education.
- Strong interpersonal, organizational, and time management skills.
- Ability to meet the physical demands of the position.
- Satisfactory employment and attendance record.
