Job Summary

Humber River Health is seeking a highly skilled and compassionate Social Worker, MSW to join our Mental Health & Addictions Program. As a key member of our team, you will play a vital role in providing high-quality patient care and support to individuals and families affected by mental health and addiction issues.

Key Responsibilities

  • Conduct comprehensive assessments and develop individualized treatment plans to address the unique needs of patients.
  • Provide evidence-based psycho-social interventions, including individual and group therapy, to promote optimal functioning and well-being.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
  • Develop and implement discharge plans, connecting patients with community resources and support services as needed.
  • Maintain accurate and timely documentation of patient progress and outcomes.
  • Participate in quality improvement initiatives and contribute to the development of best practices in mental health care.

Requirements

  • Master of Social Work (MSW) degree from an accredited institution.
  • Current membership with the Ontario Association of Social Workers (OASW) and the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Minimum 2 years of experience in mental health and/or addiction settings, with a strong understanding of evidence-based practices and treatment modalities.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to work independently and as part of a team, with a commitment to collaboration and continuous learning.
