**Position Description**:
**Social Worker, MSW - Pain Program**

**Overview**

The ambulatory pain program at SickKids, including the Chronic and Transitional Pain Clinics, is an interdisciplinary outpatient pain program providing comprehensive pain management to children, teens and their families. The program provides outpatient treatment through a functional rehab approach, including pharmacology, physical therapy and psychological/mind-body treatments for children with Chronic Pain. As well as providing extended pain management care to children, teens and their families who are recently discharged from the hospital or in preparation for, or following painful procedures, such as surgery. The pain program is supported by a high-functioning and collaborative interdisciplinary team including anesthesiologist, psychologists, nursing, physiotherapy, occupational therapy and support staff. The Social Worker of the ambulatory pain program will support patients and families across the care trajectory, from assessment, treatment, follow-up and discharge, using child and family centered care model.

**Here's What You'll Get to Do**
- Conduct and inform interprofessional family assessments and family therapy treatment for children and youth with or at risk of developing chronic pain and concurrent physical and mental health conditions.
- Implement and deliver evidence-based, trauma informed, treatment plans, inclusive of parenting skills, family counselling and therapy; support and teach relevant patient and family coping skills; and provide support around adaptation, treatment adherence and enhancement of quality of life.
- Provide treatment through various modalities including group and family therapy, individual therapy, consultation, case navigation/management, crisis intervention, community outreach and advocacy.
- Practice from an integrated model that considers the implications of medical, physical, mental health, family cultural contexts and issues of social complexity.
- Demonstrate skills in patient and family engagement including interviewing with individuals, parents, and families.
- Identify strengths and risks within families that should be considered in the context of the assessment formulation and treatment recommendations and work with caregivers to prepare for transitions between phases of treatment, discharge and transfer of care.
- Collaborate with a large and diverse inter-professional health care team.
- Navigate and secure resources that address barriers to care related to the social determinants of health.
- Work collaboratively with community agencies and services such as CAS (Children s Aid Society), Ontario Works, Children's Mental Health agencies and community primary care providers.
- Support the academic mandate of the hospital by supervising MSW students from affiliated universities.
- Assume responsibility for all aspects of practice including accountability for standards of practice, productivity, and workload related data necessary for hospital reporting.
- As a member of the social work department, cross coverage for social work colleagues is an essential part of the role.

**Here's What You Will Need**:

- Master's degree in Social Work with minimum three years clinical experience (assessment and treatment) experience working with children. Experience in an acute or pediatric mental health care setting preferred
- Registration with the Ontario College of Social Workers and Social Service Workers
- Training and experience with evidence-based treatment models inclusive of Family Therapy (Family Systems or Structural Family Therapy), Treatment of Trauma / PTSD (TF-CBT) Emotion Focused Therapy, and DBT (Dialectical Behavior Therapy). Familiarity with the complexities of chronic concurrent physical health conditions (e.g., Neurological, Orthopedic, Rheumatological) and their impacts on the mental health of children, youth, caregivers, and siblings.
