About the Role:
Closing the Gap Healthcare Group is seeking a highly skilled and compassionate Care Team Coordinator to join our team. As a key member of our team, you will play a vital role in ensuring the delivery of exceptional client-centered care.
Key Responsibilities:
- Coordinate visits and communication with our Providers to ensure seamless care delivery.
- Provide empathetic client service and administrative support to ensure efficient operations.
- Assess customer requests and address them promptly, escalating as necessary.
- Schedule and coordinate service providers to meet client needs.
- Triaging calls to ensure effective call management and provide support to internal and external customers.
Requirements:
- Outstanding commitment to providing excellent client-centered customer service.
- Effective verbal and written communication skills.
- Precise attention to detail and efficiency in data entry.
- Knowledge of Medical terminology is an asset.
- Computer and phone literacy.
- Team-oriented and able to work effectively with other team members.
