Care Team Coordinator Job Description

Closing the Gap Healthcare Group is seeking a highly motivated and detail-oriented Care Team Coordinator to join our team. As a Care Team Coordinator, you will play a vital role in ensuring the quality of care provided by our organization.

Key Responsibilities:
  • Coordinate visits and communication with our Providers to ensure seamless care delivery.
  • Provide empathetic client service and administrative support to maintain accurate records.
  • Schedule and coordinate service providers to meet client needs, ensuring timely and effective care.
  • Triaging calls to ensure effective call management and provide support to internal and external customers.
  • Perform data entry requirements for all clients using our computer system.
Requirements:
  • Passion for customer service and a desire to expand your career in the healthcare industry.
  • Effective verbal and written communication skills, with the ability to work with diverse clients and stakeholders.
  • Precise attention to detail and efficiency in data entry, with knowledge of medical terminology an asset.
  • Computer and phone literacy, with the ability to work in a fast-paced environment.
  • Team-oriented and able to work effectively with other team members to achieve common goals.
