As a Patient Care Coordinator, you will evaluate and identify patient care requirements and eligibility, facilitate access to community resources, and collaborate with patients, their families, and other healthcare professionals.

Whether your work is conducted in an office, within the community, or in a healthcare setting, you will take a pivotal role in delivering connected, accessible, and patient-focused care, supported by a collaborative team of over 9,000 regulated healthcare professionals.

Key Responsibilities:

Client Engagement – Conduct assessments for referred clients and build supportive relationships with eligible clients and their families, respecting their values and preferences.

• Comprehensive Assessment – Execute thorough assessments to identify the need for crisis intervention and/or preventive measures.

• Care Planning – Develop and implement a care and service plan that aligns with the client's needs, preferences, and available resources, in collaboration with the client and other healthcare professionals.

• Resource Coordination – Collaborate with a network of service providers to ensure comprehensive care.

• Service Execution – Initiate and oversee the implementation of the care plan.

• Monitoring and Review – Regularly assess and update the care plan to meet quality standards and desired outcomes.

• Transition Planning – Manage the discharge process and direct clients and families to alternative resources as necessary.

Required Qualifications:

Registered Nurse (BScN or diploma) with registration from the College of Nurses of Ontario; or Degree or diploma in Physiotherapy with registration from the College of Physiotherapy of Ontario; or Degree or diploma in Occupational Therapy with registration from the College of Occupational Therapy of Ontario; or Degree in Social Work with registration from the Ontario College of Social Workers and Social Service Workers; or Degree in Speech Therapy with registration from the College of Audiologists and Speech-Language Pathologists of Ontario; or Degree in Foods and Nutrition or equivalent with registration from the College of Dietitians of Ontario. A minimum of 2 years of relevant professional experience is required. Compliance with the mandatory COVID-19 vaccination policy is necessary.

Preferred Skills:

Familiarity with the healthcare delivery system and community resources in the region. Understanding of medical conditions and cognitive/psychiatric disorders. A client-centered, multidisciplinary approach to care. Proficiency in using technology for healthcare processes.

Work Schedule:

Standard hours are Monday to Friday, 8:30 AM to 4:30 PM (35 hours/week).

This role is temporary in nature.
