We are seeking a highly skilled and compassionate Registered Nurse to join our team at Fraser Health. As a Geriatric Nurse Specialist, you will play a vital role in providing high-quality care to our elderly patients and their families.
Key Responsibilities- Assess and plan care for geriatric patients in their homes, communities, and mobile clinic settings.
- Collaborate with interprofessional care teams, including physicians, to develop and implement comprehensive care plans.
- Provide education and support to patients and caregivers on geriatric health issues, care plans, and illness prevention.
- Coordinate home care services and referrals to community agencies as needed.
- Document assessments, nursing diagnoses, interventions, and outcomes in accordance with practice standards and Fraser Health procedures.
- Graduation from an approved school of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two years recent, related nursing care experience working with the geriatric population, or an equivalent combination of education, training, and experience.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
- Experience working in a geriatric care setting.
- Knowledge of geriatric care principles and practices.
- Ability to communicate effectively with patients, families, and healthcare teams.
