Geriatric Care Specialist

We are seeking a highly skilled Registered Nurse to join our team at Fraser Health. As a Geriatric Care Specialist, you will be responsible for providing person-centered care to clients in their homes, community, and/or mobile clinic setting.
Key Responsibilities- Assess physical, emotional, and psychosocial needs of clients and their caregivers
- Develop and implement comprehensive care plans in collaboration with the interdisciplinary care team
- Provide geriatric nursing services, including teaching clients and caregivers about health issues and illness management
- Collaborate with members of the interprofessional care team, including physicians, to identify and resolve issues
- Document assessments, nursing diagnoses, and interventions according to practice standards and Fraser Health procedures
- Graduation from an approved school of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
- Two years recent, related nursing care experience working with the geriatric population
- Valid BC Driver's License and access to a personal vehicle for business-related purposes
