Practical Nurse Specialist

Title: Licensed Practical Nurse.
Position Status: Full-time, 37.5 hours per week.
Location: Phoenix Center.
Reports to: Senior Manager, Clinical Operations & Services.
Compensation: $34.17 per hour.
Job Overview:
The Licensed Practical Nurse (LPN) at Phoenix Society collaborates with an interprofessional team to deliver healthcare services to clients facing physical and/or mental health challenges, substance dependence, concurrent disorders, and chronic illnesses. The LPN is tasked with the planning, execution, and assessment of nursing care within a collaborative clinical framework, adhering to the Standards of Practice set forth by the British Columbia College of Nurses and Midwives (BCCNM), while aligning with the mission, vision, and values of Phoenix Society. This is a Full-Time rotating position (06:00-18:00, 18:00-06:00).
Key Responsibilities:
Client Care:
Assist healthcare providers and team members during various examinations, treatments, and diagnostic procedures. Execute nursing tasks such as measuring vital signs and conducting urine tests as directed. Relay patient information to the appropriate supervisor and healthcare professionals. Support supervisors in managing clients with unpredictable outcomes or complex care needs. Contribute to the creation and adjustment of personalized care plans by identifying client priorities and suggesting necessary modifications. Monitor client progress in relation to expected outcomes and evaluate the effectiveness of nursing interventions. Assess clients' strengths and limitations, addressing their physiological, psychological, social, and spiritual needs. Collaborate with the treatment team to assist clients in meeting their program requirements. Review medication administration records and consult with supervisors or physicians regarding any concerns. Administer medications according to care plans and physician orders. Ensure proper storage of client medications in compliance with policies. Build therapeutic relationships with clients.Administrative Duties:
Maintain accurate computerized and paper records, ensuring proper documentation of results and consultations.Health & Safety:
Stay informed about current Occupational Health & Safety guidelines. Utilize supplies and equipment in accordance with safety standards. Handle and dispose of hazardous materials following established procedures. Report unsafe practices or professional conduct in line with organizational policies. Participate in incident investigations as necessary. Respond to incidents and provide nursing care in emergencies. Deliver first aid in accordance with Occupational First Aid Level 1 standards. Complete necessary reports for incidents and first aid. Assess client risk factors and integrate safety strategies into care plans.Qualifications:
Post-secondary diploma in nursing or a related field. Current registration as a Registered Nurse or Registered Psychiatric Nurse with BCCNM. 1 to 3 years of experience in trauma-informed acute or community health nursing. Successful completion of a criminal records check is required. Completion of Tuberculosis Screening and Employee Immunization Record is required. First Aid Level 1 and current CPR certification are mandatory. Valid driver's license is required.Skills & Knowledge:
Strong understanding of community health nursing principles, including mental health and addictions. Familiarity with BCCNM's Standards for Nursing Practice. Knowledge of the BC Mental Health Act and community resources. Proficient in conflict resolution and mediation techniques. Competent in using computerized patient care information systems.
