Job Summary

The Government of Nunavut is seeking a highly skilled RN Case Manager to join our team in Iqaluit. As a RN Case Manager, you will be responsible for coordinating and managing care for patients in the Qikiqtaaluk region, working closely with healthcare professionals, facilities, and services.

Key Responsibilities
  • Coordinate and manage care for patients in the Qikiqtaaluk region, ensuring timely access and coordination of care.
  • Work collaboratively with healthcare professionals, facilities, and services to deliver effective care.
  • Make decisions and recommendations regarding complex case management coordination, affecting the quality of care, patient satisfaction, and costs of services.
  • Develop and implement care plans, ensuring that patients receive the necessary support and resources.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • Bachelor or Diploma in Nursing, plus three (3) years of recent experience in a broad-based acute care setting.
  • Current BCLS (basic Cardiac Life Support) qualification.
  • Eligibility to register with RNANTNU.
  • Highly sensitive position, requiring a satisfactory Criminal Record Check and Vulnerable Sector Check.
Preferred Qualifications
  • Fluency in Inuktut, English, and French.
  • Knowledge of Inuit communities, culture, land, Inuit Qaujimajatuqangit, Inuktut, and experience working in a northern cross-cultural environment.
