About the Opportunity

We are seeking a highly skilled and compassionate Patient Access Navigator to join our team at Nova Scotia Health and IWK Health. As a key member of our Mental Health and Addictions team, you will play a critical role in facilitating access to mental health services for individuals in need.

The Patient Access Navigator will be responsible for:

  • Utilizing screening tools and risk assessments to identify client needs and develop effective care plans
  • Providing brief supportive counseling and psychoeducation to clients and their families
  • Collaborating with community resources and mental health teams to ensure seamless care transitions
  • Building working relationships with clients and their support networks to promote self-navigation of the healthcare system
  • Identifying and addressing barriers to treatment and support, and connecting clients with relevant resources

The ideal candidate will possess a strong background in social work, with experience working with individuals with mental health and addiction concerns. You will have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
