Key Responsibilities
  • Assess and develop individualized treatment plans in collaboration with the clinical team
  • Coordinate and facilitate services, including counseling, education, and support groups
  • Work closely with community agencies and healthcare providers to ensure seamless care coordination
  • Provide education and support to individuals and their families on mental health and wellness
  • Participate in multidisciplinary team meetings to discuss client progress and develop care plans
Requirements
  • Minimum four years of experience working with individuals with serious and persistent psychiatric disabilities
  • Strong knowledge of psychosocial rehabilitation and recovery principles
  • Excellent communication, interpersonal, and organizational skills
  • Ability to work effectively in a fast-paced, dynamic environment
  • Valid driver's license and reliable vehicle
Preferred Qualifications
  • Master's degree in a related field, such as social work, psychology, or counseling
  • Experience working with diverse populations, including Indigenous and culturally diverse communities
  • Knowledge of community resources and services
