Care and Be Cared For - Your Home

We are seeking a highly organized and detail-oriented Bilingual Team Assistant to provide support for our assigned team in their daily activities. As a valued team member, you will play a key role in providing connected, accessible, patient-centered care.

Key Responsibilities:

  • Provide information and referral services to patients, families, service providers, and the general public.
  • Communicate with team members to ensure information is distributed and obtained appropriately.
  • Maintain an efficient, current, and secure client record, including completing entry of all required information, documenting actions taken, and ensuring file availability.
  • Process requisitions, prepare related documents, and monitor inventory of supplies.
  • Update client files by entering information corresponding to standardized pathways or as per Care Coordinator direction.
  • Liaise with staff, care providers, clients, and pharmacies regarding confirmation of dates, deliveries, discrepancies, and delays of supplies.
  • Book routine client home visit appointments and client/family conferences.
  • Arrange client transportation by taxi or ambulance, as authorized by Care Coordinators.
  • Maintain team in/out schedule.

Requirements:

  • High school graduation diploma plus.
  • Successful completion of 1 year Community College diploma in Office Administration, Business, Health Administration, or equivalent program.
  • Minimum 18 months previous experience in similar customer service or related position in the health, social, or business administration fields.
  • Fluency in English and French is required.
