Community Health Support Specialist

Island Health is seeking a dedicated and compassionate Community Health Worker 2 to join our team.
In this role, you will play a vital part in supporting the well-being of individuals within our community. Your responsibilities will encompass providing direct care, assisting with daily living activities, and connecting clients with essential resources.
Responsibilities:
- Deliver personalized care services to clients in their homes or community settings.
- Assist with personal hygiene, medication reminders, and other daily tasks as needed.
- Provide emotional support and companionship to clients.
- Connect clients with appropriate healthcare providers and community resources.
- Maintain accurate records of client care and progress.
Qualifications:
To be successful in this position, you will require:
- A Home Support/Resident Care Attendant/Health Care Assistant Certificate from a recognized post-secondary program or an equivalent combination of education, training, and experience.
- A valid Food Safe Certificate and a current BC driver's license.
- Current registration and good standing with the BC Care Aide & Community Health Worker Registry.
Skills and Abilities:
- Ability to work independently with minimal supervision.
- Excellent communication skills, both written and verbal.
- Proven ability to build rapport and effectively interact with clients from diverse backgrounds.
- Physical stamina and the ability to perform the essential duties of the role.
- Strong organizational and time management abilities.
