We are seeking a highly skilled Registered Practical Nurse to join our Assertive Community Treatment Team (ACTT) in Southwest Ontario. As a key member of our team, you will play a vital role in providing high-quality care and support to individuals with serious and persistent mental illness.
Key Responsibilities- Conduct comprehensive mental and physical health assessments to identify client needs and develop individualized care plans.
- Administer medications within your scope of practice and in consultation with the attending ACTT psychiatrist.
- Collaborate with the ACT team to provide follow-up nursing care and monitoring to clients being referred to the Clozapine Clinic.
- Provide professional support and education to clients and their families, using an empowerment/recovery approach.
- Work closely with the clinic psychiatrist to monitor and provide follow-up care for clients transitioning out of ACT services.
- Licensed Registered Practical Nurse (Dipl. N.)
- Member in good standing with the Ontario College of Nurses
- Four or more years' experience as an RPN, preferably in a mental health setting
- Excellent health assessment skills, sound clinical judgment, and diagnostic skills
- A second language (French, Cantonese, Mandarin, Farsi, Italian, Korean, Punjabi, Russian, Tamil, Urdu) reflecting the local community is preferred
- Knowledge of the Ontario Mental Health Act, mental health reform principles, the Substitute Decisions Act, the Health Care Consent Act, and PHIPA requirements
- Ability to apply Recovery principles and empowerment-oriented philosophies and practices in work with clients
- A valid Canadian driver's license, a minimum of $1,000,000 third-party liability insurance, and proof that insurance covers the use of a personal vehicle to transport clients
- A satisfactory Vulnerable Sector Screening (Police Check)
