Job Summary:
The Registered Nurse (RN) will work collaboratively within a multi-disciplinary healthcare team on a busy acute medicine unit and a subacute unit at the Foothills Medical Centre. The patient population on these units includes acute medical patients and patients awaiting transfer to an alternative level of care settings in the community.
Key Responsibilities:
- Provide high-quality, patient-centered care to patients with complex and diverse care needs.
- Work collaboratively with a multi-disciplinary team to ensure seamless patient care.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with patients, families, and healthcare teams to ensure effective communication and coordination of care.
- Participate in discharge planning and ensure a smooth transition of care for patients.
- Contribute to the development and implementation of policies and procedures to improve patient care and outcomes.
Requirements:
- Completion of an accredited nursing education program.
- Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
- Recent Charge Nurse experience and active participation in discharge planning.
- A strong understanding of CoACT principles.
- At least 2 years of recent acute medicine nursing experience and strong clinical interest in geriatrics.
- Demonstrated ability to work and lead in a team environment.
- Demonstrated ability to work effectively with patients and families in stressful situations.
- Strong clinical and assessment skills, including patient assessment, IV certification, CVC/PICC certification, Foley insertion, and Glucometer certification.
- Excellent interpersonal communication, effective time management, and organizational skills.
- Ability to lift and turn patients safely utilizing appropriate techniques and equipment.
- Ability to lift and transfer up to 50 lbs. up to 25% of the time - utilizing appropriate equipment and techniques.
Preferred Qualifications:
- Relevant Senior's and Geriatric Health education.
- Connect Care experience.
