Job Overview

The BC Centre for Excellence in HIV/AIDS' Research & Innovation Centre is a pioneering health facility that integrates a multidisciplinary clinical team dedicated to delivering comprehensive care for clients in the Downtown Eastside.

The exceptional care provided by our clinical team emphasizes HIV/AIDS, HCV, and other infectious diseases, supported by continuous research, innovation, and wellness initiatives.


Role of the Primary Care Nurse

As a vital member of an interprofessional healthcare team, the Primary Care Nurse is responsible for assessing and engaging new clients, documenting evaluations, and developing care plans in collaboration with team members.

In partnership with colleagues, the Primary Care Nurse also facilitates team and case management meetings.


Qualifications and Skills

- Registration with the BC College of Nurses and Midwives (BCCNM) as a practicing RN or RPN.

- A minimum of two years of recent experience in healthcare settings, particularly with diverse marginalized populations, focusing on primary care, HIV, addictions, or mental health.

- Completion of relevant certifications such as Point of Care HIV Testing, BCCDC Immunization, and Narcan Certification is required, or a commitment to complete within six months of hire.

- Proven experience in performing venipuncture and knowledge of trauma-informed care, violence prevention, and harm reduction practices.


Core Responsibilities

- Conduct assessments utilizing chronic disease management principles, identifying issues and collaborating with the interdisciplinary team.

- Perform primary care nursing duties, including wound care, addiction assessments, and HIV/HCV testing.

- Document assessments, care plans, and patient progress in both manual and computerized systems.

- Ensure continuity of care by coordinating with team members and evaluating client outcomes based on their care goals.

- Act as a resource for the team, providing insights based on nursing theory and practice for clients with complex health needs.

- Lead educational sessions for clients, addressing their learning needs and promoting health-enhancing behaviors.

- Participate in nursing orientation and share knowledge with peers.

- Engage in quality improvement initiatives and contribute to the development of best practices.

- Advocate for clients' rights, ensuring dignity, privacy, and access to information.


Additional duties may be assigned as necessary.
