About the Opportunity

We are seeking a skilled Pharmacist to join our Primary Health Care team. Reporting to the Manager of Primary Health Care, you will collaborate with interprofessional healthcare providers, including family practice teams and chronic disease management teams, to deliver patient-centered care that optimizes safe, effective, and appropriate drug therapy.

Your responsibilities will encompass:

  • Working collaboratively with healthcare professionals to provide comprehensive patient care
  • Identifying and resolving drug-related problems
  • Completing medication reconciliations
  • Participating in interprofessional medical rounds
  • Providing patient education on medications
  • Making therapeutic recommendations to the healthcare team
  • Ensuring safe and effective drug therapy through participation in and oversight of drug distribution systems (inpatient and outpatient)

About You

The ideal candidate possesses:

  • A Pharmacy degree from an accredited university program
  • Completion of a Pharmacy Residency Program (preferred)
  • Current licensure to practice pharmacy in Nova Scotia
  • Minimum two years of clinical experience in a hospital pharmacy setting (preferred)
  • A strong understanding of drugs and therapeutics
  • Excellent oral and written communication skills
  • The ability to provide accurate and timely recommendations regarding safe and cost-effective drug use
  • Competencies in other languages are an asset, with French being preferred
