Pharmacist - Primary Health Care Specialist

About the Opportunity
The Clinical Pharmacist will work collaboratively with an interprofessional team to provide patient-centered care that optimizes safe, effective, and appropriate drug therapy. This includes working on selected medical services, identifying and resolving drug-related problems, completing medication reconciliation, participating in interprofessional medical rounds, providing patient education, and making therapeutic recommendations to the team.
The Pharmacist is responsible for ensuring safe and effective drug therapy by participating in and overseeing drug distribution systems (inpatient and outpatient) as well as contributing to the effective and efficient attainment of the goals and objectives of Nova Scotia Health Pharmacy Services.
This position provides coverage to other locations within Yarmouth County as required.
About You
We are seeking a highly skilled and experienced Pharmacist to join our team. The ideal candidate will have a strong knowledge of drugs and therapeutics, effective oral and written communication skills, and the ability to make accurate and timely recommendations regarding safe and cost-effective use of drugs.
