Case Counsellor Specialist

Salary: J- $30.11

Program Overview – Homeward Bound & Neighbourhood Programs
This program encompasses various elements, including career advancement, life skills development, academic enhancement, and computer training, alongside a college diploma initiative. Additionally, it features an internship segment and access to sustainable employment pathways. Homeward Bound also provides on-site childcare, family assistance, and after-school programs for children.

Key Responsibilities

  1. Utilize professional case management techniques and diverse clinical approaches to execute casework responsibilities, which include client assessment, goal establishment, action plan formulation, ongoing intervention, re-evaluation, case closure, and follow-up.
  2. Facilitate client engagement in all facets of the Homeward Bound program, collaborating with Community Colleges and academic enhancement providers to guarantee participant enrollment and active involvement in programs and practical training experiences such as internships.
  3. Ensure clients focus on achieving program objectives and milestones.
  4. Coordinate with Toronto Children Services and Toronto Employment and Social Services.
  5. Provide essential support to assist clients in overcoming daily living challenges, advocate for their needs, and help them access relevant support services such as healthcare, financial management, and housing assistance.
  6. Identify learning obstacles and refer participants to suitable support resources.
  7. Address emergencies, deliver crisis intervention, and manage a caseload.
  8. Maintain client documentation in a centralized database according to program policies.
  9. Gather statistical information in line with central and unit policies. Assist in ensuring the safety of children during off-site excursions and in on-site activities.
  10. Adhere to all pertinent legislation, by-laws, program, and organizational policies.

Qualifications

  1. Master of Social Work or a Master's Degree in a related discipline. Candidates with a BSW and relevant experience will also be considered.
  2. Registration with the Ontario College of Social Workers and Social Service Workers or its equivalent is mandatory.
  3. A minimum of two (2) years of relevant experience working with women facing homelessness, violence, or related challenges at a case management level, including employment planning and preparation.

Preferred Skills

  1. Extensive experience in various counselling techniques, particularly with women who have faced homelessness or domestic violence.
  2. Proven case management and group facilitation abilities.
  3. Strong understanding of career planning and job search strategies.
  4. Provide continuous feedback to improve the employability of all clients.
  5. Demonstrated intervention skills, including crisis management.
  6. Commitment to ongoing self-reflection to ensure best practices in service delivery.
  7. Social assessment capabilities.
  8. Knowledge and skills in conflict resolution.
  9. Well-developed documentation skills.
  10. Proficient computer skills, including internet research.
  11. Familiarity with community development and referral to social resources.
  12. Essential collaboration and teamwork skills.
  13. Able to perform under pressure.
  14. Experience working with clients from diverse backgrounds.
  15. Understanding of anti-oppressive practices and trauma-informed care.
  16. Lived experience as a member of the BIPOC community or other experiences of social identity intersectionality.
