Closing the Gap Healthcare Group is seeking a skilled and compassionate Visiting Nurse to join our team. As a Visiting Nurse, you will provide high-quality patient care in the comfort of clients' homes, focusing on wound care management and other specialized services.
Key Responsibilities:- Deliver comprehensive wound care management, including IV antibiotic administration, IV Cannulation, Chemo disconnects, CVAD care, and PORT-A-CATH care access/de-access.
- Collaborate with the HCCSS (LHIN) to ensure seamless care coordination.
- Travel between clients to provide care, utilizing your excellent communication skills to build strong relationships.
- Document patient progress, create reports, and update records as needed.
- Order supplies for patients, ensuring timely delivery of essential care items.
- Current certification as a Registered Nurse (RN) or Registered Practical Nurse (RPN) with the CNO.
- Standard First Aid & CPR/ AED (Level C) certification.
- Mask Fit test certification.
- Minimum of two years' community or clinical/hospital experience.
- Ability to work weekends and be on-call.
