Job Summary

We're seeking an experienced Occupational Therapist to join our team as a Home HealthCare Specialist. As a key member of our team, you'll work with diverse client groups across Southern Ontario and the capital region, providing consultation, assessment, and planned interventions to support their independence and well-being.

Key Responsibilities

  • Assess and implement occupational therapy interventions in accordance with professional practice guidelines and standards.
  • Develop and implement client-centered care plans, working collaboratively with clients, families, and other healthcare professionals.
  • Ensure coordinated delivery of care through effective communication and collaboration with the client's care team.
  • Document all assessment findings, treatment plans, progress, and discharge summaries in accordance with professional standards and organizational requirements.
  • Support safe client care through ongoing professional development and adherence to VHA's principles of continuous quality improvement.

Requirements

  • Master's degree in Occupational Therapy from a recognized institution.
  • Member in good standing with the College of Occupational Therapists of Ontario.
  • Valid driver's license with access to a vehicle.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong organizational and time management skills.
  • Proficiency in electronic documentation and virtual care.
