Job Title: Care Coordinator
Job Summary:
The Care Coordinator is a vital member of our team, responsible for providing exceptional patient care and administrative support to our clinic. As a Care Coordinator, you will be the first point of contact for patients, answering their questions, and addressing their concerns with a high level of professionalism and empathy.
Key Responsibilities:
- Provide administrative support to the Care Coordinator Manager and Clinic Director
- Coordinate patient intake, data entry, and scheduling of appointments
- Collect payment at time of service and track coverage for all funding streams
- Complete data entry and billing, and coordinate courier packages
- Provide excellent customer service and communicate effectively with patients, staff, and external customers
Qualifications & Core Competencies:
- High School Diploma, G.E.D. or equivalent
- Excellent customer service skills and telephone etiquette
- High degree of organizational skills and ability to prioritize and multi-task
- Excellent communication (verbal/written) and interpersonal skills
- Strong computer and data entry skills
- Flexible and adaptable to change
